What this support looks like

Individualized home supports with training combine home-based support with structured teaching. Staff do not only help complete tasks — they coach, model, and practice the steps with the person so that skills are gained and retained over time.

Examples of daily activities

  • Practicing a routine step by step until it can be done independently
  • Learning to use a new household tool or adaptive equipment
  • Practicing money handling with real purchases
  • Coaching through making a phone call or booking an appointment
  • Reviewing progress together and adjusting goals

These are illustrations, not a fixed list. Support is planned with the person and may differ from what's shown here.

Person-centered benefits

  • Skills are built deliberately, not left to chance
  • Progress can be documented and shared with case managers
  • Support gradually steps back as confidence grows
  • The person gains control over their own routines
