Who we are

Lakeland Health Services is a Minnesota Home and Community-Based Services provider serving individuals and families in and around Coon Rapids and Fridley. We are a 245D licensed provider, and our work is grounded in a simple belief: people thrive at home, in their own communities, with the right support around them.

We serve individuals with disabilities and older adults who want to keep living in their own homes and stay connected to their communities — and the families, guardians, and case managers who support them.

Our mission

To provide person-centered home and community-based services that help individuals live safely and independently, with dignity and choice in every part of daily life.

Our vision

A community where every person, regardless of ability or age, has the support they need to live the life they choose — connected to family, neighbors, and the places they call home.

Our Approach

Person-centered, in practice.

Person-centered means the person leads. We begin with what someone wants their days to look like, then match our support to that picture. People are never asked to fit a program; support is shaped around the person, their culture, their routines, and their relationships.

  • We start by asking what a good week looks like for this person.
  • We build schedules around routines, culture, and preferences — not staff convenience.
  • We support people to do things themselves for as long as they safely can.
  • We involve family, guardians, and case managers as partners, with the person's direction.

Core Values

What we hold ourselves to.

Dignity

Every person deserves to be treated as an adult with a history, preferences, and a voice — never as a task to complete.

Choice

Support looks different for everyone. We build schedules and routines around what the person actually wants.

Safety

Independence is only meaningful when someone feels secure. We plan carefully, document clearly, and stay reachable.

Relationships

Consistency matters. We work to keep the same caregivers showing up, because trust is built visit by visit.

Stewardship

We treat the trust families and case managers place in us as something to protect, in every detail of our work.

Dignity and independence

Independence is made of small things: choosing what to eat, deciding when to shower, answering your own door, keeping your own routines. Our caregivers are trained to notice those moments and protect them, offering help without taking over.

That also means respecting privacy, knocking before entering, asking before assisting, and never discussing one person's business with another.

Community involvement

Our teams live and work in the same communities we serve. That means familiar neighborhoods, shorter drives to appointments, and caregivers who know the local grocery store, the library, and the park down the street. We also partner with case managers, guardians, and local organizations so support feels coordinated rather than fragmented.

Leadership

The people behind the support.
