24-Hour Emergency Assistance

Sometimes support is needed the moment it is needed, not on the next business day. Our 24-hour emergency assistance gives individuals and families a number to call when something unexpected happens at home.

What this support looks like

This service provides on-call assistance for urgent, non-medical situations in the home. A trained staff member responds by phone to understand the situation and, when appropriate, comes to the home to help. Support may include assisting someone who has fallen to get up safely, responding to a household emergency, or helping coordinate the right non-medical response. Medical emergencies should always be directed to 911.

Examples of daily activities

  • Responding to an after-hours call when someone needs help getting up after a fall
  • Helping resolve an urgent household problem, such as a water leak or a broken appliance
  • Checking in when a person feels unwell and connecting them with the right resource
  • Coming to the home when a scheduled caregiver cannot and support is still needed

These are illustrations, not a fixed list. Support is planned with the person and may differ from what's shown here.

Person-centered benefits

  • Families have a plan and a number to call instead of wondering who to reach
  • Each response is documented, so patterns can be noticed and routines adjusted
  • Individuals can remain at home knowing help is reachable
  • Reduces unnecessary emergency department visits for non-medical situations

Eligibility for this service is determined through a person's assessment, authorization, and funding, typically coordinated with a county or tribal worker and a case manager. Lakeland Health Services does not guarantee waiver eligibility, payment coverage, or specific outcomes.
