What this support looks like

Individualized home supports with family training provide home-based support while also training the family members who are part of the person's support network. The aim is a consistent, coordinated approach at home — the same strategies, language, and expectations, applied by everyone involved.

Examples of daily activities

  • Coaching family members on a routine or communication approach
  • Practicing a skill together with the person and their family
  • Reviewing what is working and adjusting the support plan
  • Modeling techniques for encouraging choice and safety
  • Planning how responsibilities are shared across the week

These are illustrations, not a fixed list. Support is planned with the person and may differ from what's shown here.

Person-centered benefits

  • One consistent approach instead of mixed messages
  • Family members feel more confident and less alone in the work
  • Support can continue smoothly when staff are not present
  • Plans stay aligned with the person's own goals
