Individualized Home Supports (IHS)

No two homes run the same way. Individualized home supports are planned around the person's own routines, goals, and the specific areas where help makes the biggest difference.

What this support looks like

Individualized home supports provide training, assistance, and supervision in a person's own home or family home, based on assessed needs and goals. Support may address communication, community living, health and safety, household management, and other daily living areas.

Examples of daily activities

  • Support with morning and evening routines
  • Meal planning, preparation, and kitchen safety
  • Household tasks and keeping the living space organized
  • Support with communication, appointments, and paperwork
  • Practicing safety skills and responding to everyday problems

These are illustrations, not a fixed list. Support is planned with the person and may differ from what's shown here.

Person-centered benefits

  • Support shaped around the person instead of a fixed program
  • Consistency in routines that the person can rely on
  • Skills practiced in the actual home environment
  • Families and case managers see the same plan carried out
