What a 245D license actually means for your family
You will see “245D licensed” on nearly every Minnesota provider website. Here is what the license covers, and what it does not promise.

If you have looked at more than one Minnesota home care website, you have seen the phrase “245D licensed provider.” It is worth understanding, because it tells you something real — and it does not tell you everything.
245D is the Minnesota statute that governs Home and Community-Based Services for people with disabilities and older adults. A provider licensed under 245D has agreed to a set of requirements around how services are planned, documented, staffed, and supervised. That includes person-centered planning, written service delivery plans, staff training and background checks, incident reporting, and licensing reviews by the Department of Human Services.
In practical terms, that is a meaningful safeguard. It means the agency is accountable to a state regulator, not only to the family hiring them.
What the license does not do is guarantee that a specific service is authorized for a specific person. That depends on the person's assessment, their waiver or funding, and their county or tribal worker's authorization. A provider can be fully licensed and still be unable to serve someone whose needs fall outside what has been authorized.
It also does not guarantee quality in a way you can take for granted. Licensing sets a floor, not a ceiling. Families should still ask how caregivers are matched, how often plans are reviewed, who answers the phone at 9 p.m., and what happens when a scheduled caregiver cannot come.
A useful way to think about it: the license tells you the agency has agreed to the rules. The conversations you have with them tell you how they actually work.
