Welcome to our new website

We have rebuilt our website with one goal: making it easier to find out whether we are the right fit, and easier to reach us if we are.

You can now read about each of our 245D services in plain language — what the support actually looks like day to day, and what to ask before you begin. Every service page includes a way to start a referral without hunting for a phone number.

Both of our offices are listed with addresses, directions, and click-to-call numbers, so you can reach the location closest to you. Our team answers questions about services, referrals, and employment from either office.

For case managers, there is a dedicated referral interest form that asks for the general details we need to follow up — and nothing more. We deliberately do not collect diagnoses, identification numbers, or records through this website. When sensitive information is needed, we will arrange an appropriate way to exchange it.

We will also use this space for care tips, community news, and announcements. If there is a topic you would like us to cover, tell us — this site is meant to be useful, not decorative.
